The Data Behind the Disconnect

Plans design benefits for members. Physicians write prescriptions for patients. Between those two moments, something breaks down, and it shows up in every abandoned prescription, every confused phone call, and every dollar of avoidable spend.

For four years, we asked thousands of members how they experience high drug costs. This year, we conducted a new survey with just physicians, asking how they manage treatment affordability. This webinar puts both answers on the same stage.

The member survey

Four years running

Thousands of members, asked every year how high drug costs actually reach them. What they pay. What they expected to pay. What they did about it.

The physician survey

New for 2026

Prescribers only, asked how they manage treatment affordability from inside the exam room. The first time we have run both sides of the prescription.

Three findings. Thirty minutes.

Each one is the same question asked twice, once in the exam room and once at the pharmacy counter. The distance between the two answers is the story.

The Price Hunt

Members and physicians are chasing the same thing from opposite ends, and both keep hitting walls.

In the exam room

Nearly 2 in 3

physicians have had a poor experience with real-time benefit tools, or have never encountered one.

At the counter

55%

of members who search an app or website for a lower-cost option say it is difficult to find one.

See where the search for the lowest-cost option breaks down on both sides of the prescription.

Flying Blind

Neither side can see the price before it lands. The surprise arrives twice, once in the exam room and once at the register.

In the exam room

92%

of physicians are surprised by what their patient actually pays at the pharmacy.

And 84% of practices field five or more high-cost prescription calls every week.

At the counter

  1. Can they predict the price? 53.1% say it is confusing to work out what the final price will be.
  2. Do they trust the price? 55.6% don’t believe they are always paying the lowest one. Among members whose plan offers no savings program, that climbs to 63.1%.

See what both sides say about managing sticker shock.

The Missing Switch

Here the two sides stop disagreeing. They want the same thing, in almost the same numbers. Nothing is carrying it across.

In the exam room

96%

of physicians are already comfortable switching a patient to a lower-cost, clinically appropriate alternative.

At the counter

84%

of members want their provider making that call for them, automatically.

See what’s stopping two aligned groups from closing the gap today.
